FPBM collects personal information provided by our clients, to effectively manage our business. We collect only the minimum information we need so that we can effectively contact you and help you with your companion animal.
What we collect: We may collect the following information: * Your Name and Address * Your Phone Numbers and E-mail Address * The Names of All members of your household who have contact with your animal * The Ages of any members of your household, under the age of 16 years * Your Animals Details, including: Name, Age, Gender, Current Health Status, Record of Training, Behaviour, Advice and Progress * The Name and Contact Details of Your Animals Veterinary Surgeon
What we use your personal details for: * To Contact You to make appointments and bookings * To Provide You with Information, Follow up Advice and Support * To be able to get to you for home visits * To Maintain Good and Accurate Record Keeping
Disclosure of Personal Information: FPBM will not disclose or share any personal information it holds about you without your consent unless required by law to do so.
The Quality, Accuracy and Correction of Personal Information we hold: Protecting personal information is a priority for us and FPBM always considers security and confidentiality when handling personal information. It is important that the information we have and use is accurate, complete and up to date. If at any time, your personal details change, please let us know so we can update our records.
How long we keep hold of your information for: We keep your information for as long as it is required to provide a service to you. We delete your information from our systems 3 years after your last contact with us.
Access to Personal Information: We respect your right to access personal information FPBM holds about you You have various rights regarding the data we hold about you: * Right of access (to receive a copy of your personal data) * Right to rectification (to request inaccurate data is corrected) * Right to erasure (to request that data is deleted) * Right to restrict processing (to request we don't use your data in a certain way) * Right to object (to make a complaint about any aspect of our use of your data)
If we are processing your information based on you giving us consent to do so, you have the right to withdraw your consent at any time. Doing so may mean we are unable to provide the service you are hoping to receive and the implications of you giving or withdrawing your consent will be explained at the time.

Data Security: FPBM is committed to ensuring that your information is secure and takes reasonable steps to protect all personal information we hold, from misuse, loss and unauthorised access, modification or disclosure. All data and laptops are kept in a secure locker and laptops are password protected. We will take reasonable steps to destroy or permanently de-identify your personal information when it is no longer needed.
